Loose or fixed braces – which treatment method to choose?

The decision for a removable or fixed appliance depends on the severity of the malposition and the growth phase of the child/adolescent. In most cases, removable appliances, i.e. loose braces, are started during the growth phase. The loose brace first corrects the jaws to each other – forebite, backbite and crossbite. The fixed brace then straightens the teeth in a further step.

Removable appliances

Removable appliances are also colloquially often referred to as "loose" braces. They are usually used in childhood or adolescence to influence bone growth. They can also be used to improve space conditions. Removable plates can be used in orthodontics to correct misalignments of one or both jaws concerning each other.

They are used, for example, in:

  • Forward and backbites
  • Lateral crossbites
  • Open bites

Loose braces are used as active treatment devices, especially during growth.

Fixed appliances

With fixed braces, also known as multibrackets, the teeth are moved by a wire. The fixed appliances are used when the movements cannot be performed by removable appliances, e.g. in the case of:

  • Turning of a tooth
  • A larger gap closure
  • Lengthening or shortening of a tooth

Thus, in orthodontics, it has to be decided from case to case whether removable and/or fixed treatment is necessary.
